Programs & support services

At Evolve Housing, we design our programs by listening to our residents and understanding the support you need. We gather feedback through surveys, our Resident Advisory and Maintenance Advisory Groups, and other resident initiatives.

By listening to you, we’ve created a range of programs and services aimed at improving your health, wellbeing, social connections, and helping you take steps toward independent living.

Support Pathways Program

The Support Pathways Program helps Evolve Housing residents stay on track with their tenancies and improve their wellbeing. We know that residents may need help for various reasons, including:

    • Health and wellbeing

    • Property care

    • Financial challenges (NILS)

    • Accessing support (My Aged Care, NDIS)

    • Family support

    • Advocacy, and more

To get support, you can contact us directly or ask your Housing Manager for a referral. Our team will work with you to create a personal Support Plan to meet your goals. We aim to make sure you have the right services in place so you can confidently manage your tenancy.

Joanne’s story

Joanne* came across the program while visiting a Department of Communities and Justice (DCJ) office in Liverpool. She was dealing with depression, anxiety, and trauma, and living in community housing. Despite her challenges, she had a dream of working at DCJ, but didn’t think it was possible.

With the help of Evolve Housing’s Case Coordinator, she developed a resume, improved her interview skills, and received support like financial assistance for fines and travel. She applied for a role at DCJ, and after several interviews, was offered a full-time position as a Housing Officer. She has now been employed for over 12 months and has been offered opportunities to act in senior roles, showing how this program can empower vulnerable people by building their confidence and independence.
